Although Comprehensive Women’s Health Care is a “paperless” medical office, from time to time it will be necessary to obtain signatures on various forms including: privacy rights, operative consents, etc.

In the near future, we hope to incorporate an electronic signature capture that will allow us to be free from these forms. For the time being, below is a list of the common forms required at the time of your visit. Our staff will attempt to inform you of the forms needed prior to your appointment. For those that are able, print these forms at you home/office and complete them prior to arriving at the office. The staff will then be able to scan the document with the signature and shred the original. All documents will be fully explained and all questions will be answered, prior to those documents being scanned into the medical record.
